Sending Your Samples Back
Register online, pack, post – the lab does the rest.
Our online Mould Sample Return Form replaces the printed form. You register your samples in your browser before you post them: one submission per property, all samples from that property in one report. You receive a confirmation e-mail with a copy of your submission for your records – no paper, no illegible handwriting, nothing to re-type at the lab.
Where is the form? The link is personal to your order: you find it in your order e-mails. It is not published here, so only customers with an order can register samples. Can't find it? Contact us and we send it again.
Three steps
1. Register online
Open the return form from your order e-mail, enter your order number and contact details, the property, then each sample with its barcode. Takes about five minutes.
2. Pack
Both caps closed on every cassette, labels legible, everything in the padded, prepaid return envelope from the pack.
3. Post
The envelope is prepaid and addressed to the lab. Post it the same day if you can – the sooner the samples arrive, the sooner you have your report.
What the form asks
Your details
Order number, company, the contact person the confirmation is addressed to, e-mail address, phone and postal address.
Property
Property name and address as they should appear on the report, the sampling date and who took the samples.
Samples
Air samples start with the outdoor reference, then one indoor sample at a time: location, the 8-digit barcode from the cassette, sampling time (5 minutes) and flow rate (15 l/min). Surface swabs are added in their own section – both can go in one submission.
Shipment
Date posted and the number of samples in the envelope. Submit – the confirmation follows by e-mail.
Do's and don'ts
Do
Close both caps. Label every cassette with room and time. Register before you post – the lab matches the barcodes to your submission.
Don't
Don't open a used cassette. Don't post cassettes loose or unregistered. Don't leave cassettes in a hot car for days.
Lab address

Required fields cannot be skipped in the online form, so submissions arrive complete. If a barcode does not match a cassette in the envelope, the lab contacts you before processing.
